The Trusts were established to provide benefits for eligible member and dependents through the collective bargaining process with UFCW Local Union No. 1496. This site provides general benefit information, forms and other Trust related items.

The Administration Office will provide you with tax Form 1095-B which shows whether you and your family had qualifying health coverage for part or all of 2019.

You DO NOT need Form 1095-B to file your taxes. Just like last year, you can check a box on your tax return verifying you had qualifying health coverage.

You can expect to receive your Form 1095-B for the 2019 tax year in the mail by January 31, 2020. Check to make sure it’s accurate and keep it with the rest of your tax documents.

IMPORTANT REMINDER:  All members must complete and return the required Enrollment Form to enroll for coverage in 2020.  Failure to complete and return the Enrollment Form by the due date will result in you NOT being eligible for coverage in the Health Plan beginning January 1, 2020.  Note: The deadline to return the form is December 20th.

Or you can go to www.akufcwtrust.com to print an enrollment form, click on Forms then click the drop down “Enrollment” form.
